Intake & Admissions

Alexander Youth Network provides a wide array of programs and services accessible in many locations throughout the Western, Central, and Piedmont regions of North Carolina. In the side panel on the left you can easily find Program Descriptions, Program Locations, and Contact Information.

To make an appointment with a therapist please contact a member of our Intake team calling 855.362.8470 or refer to the contact information below. We also offer walk-in assessments on a first-come, first-served basis at our Child & Family Therapy Office located at 5855 Executive Center Drive, Suite 111, Charlotte, NC 28212, the first and third Wednesday of each month from 8:30 a.m. to 3:30 p.m.

To make a referral to an Alexander treatment program or service, please contact Intake.  Review the Admissions Guidelines for reference.  Complete the Referral Form available as an Adobe PDF or an MS Word file.  Should you need it, the Release of Information is available  for download.

If you are referring a child to Alexander’s Therapeutic Foster Care service and a potential Treatment Home has been identified, please complete and submit the TFC Application Addendum (Adobe PDF or MS Word file) to secure the placement.
